1. Introduction and Acceptance of Terms
Welcome to My Hashi, a mobile application designed to help you track the progression of thyroid and Hashimoto’s diseases. My Hashi is developed and owned by Patryk Szurgot iPapi (operating under the brand relicode.com), a sole proprietorship (jednoosobowa działalność gospodarcza) registered in Poland.

By downloading, installing, or using My Hashi, you (“User”) agree to be bound by these Terms and Conditions (“Terms”). If you do not agree to these Terms, do not use the application.
2. Description of the Application
My Hashi is a personal health diary application available for iOS and Android devices. The application allows you to:
- Record and track health test results (e.g., TSH, FT3, FT4, and other thyroid-related markers)
- Log body weight over time
- Track your daily well-being and mood
- Record medications and treatment details, including drug names and dosages
Important: All data entered into My Hashi is stored exclusively on your device. The application does not transmit any personal or health data to external servers, the internet, or any third party. You have full control over your data at all times.
3. Eligibility
The application is intended for users aged 18 years and older. By using My Hashi, you confirm that you are at least 18 years old. If you are under 18, you may only use the application under the supervision of a parent or legal guardian who agrees to these Terms on your behalf.
4. Medical Disclaimer
My Hashi is not a medical device and does not provide medical advice.
The application is intended solely as a personal tracking and journaling tool. It is not designed to diagnose, treat, cure, or prevent any disease or medical condition. Nothing in the application constitutes or should be interpreted as medical advice.
Always consult a qualified healthcare professional (physician, endocrinologist, or other specialist) before making any decisions regarding your health, treatment, medication, or diagnosis. Do not disregard professional medical advice or delay seeking it because of information recorded or presented in this application.
Patryk Szurgot iPapi assumes no liability for any health decisions made based on data logged in this application.
5. License and Permitted Use
Subject to your compliance with these Terms, Patryk Szurgot iPapi grants you a limited, non-exclusive, non-transferable, revocable license to download and use My Hashi on a device you own or control, solely for your personal, non-commercial purposes.
You may not:
- Copy, modify, or distribute the application or its content
- Reverse-engineer, decompile, or disassemble the application
- Use the application for any unlawful purpose
- Attempt to gain unauthorized access to any part of the application or its underlying systems
- Remove, alter, or obscure any proprietary notices or labels
6. Purchases and Payment
My Hashi may be available for purchase or include in-app purchases through the Apple App Store or Google Play Store. All purchases are subject to the terms and conditions of the respective platform through which they are made. Refund requests are handled in accordance with Apple’s or Google’s refund policies.
Patryk Szurgot iPapi reserves the right to change pricing at any time. Continued use of the application after a price change constitutes acceptance of the new pricing.
7. Data and Privacy
Because My Hashi stores all data locally on your device and does not connect to the internet, Patryk Szurgot iPapi does not collect, process, or store any of your personal health data on its servers.
You are solely responsible for:
- The security of your device and access to the application
- Creating backups of your data using your device’s built-in backup tools (e.g., iCloud or Android Backup)
- Any loss of data resulting from device failure, loss, theft, or app deletion
Please refer to our Privacy Policy for full details on how we handle any incidental data (e.g., crash reports, app store analytics) that may be collected by platform providers.
8. Intellectual Property
All intellectual property rights in My Hashi, including but not limited to the application design, user interface, graphics, source code, trademarks, and trade name “My Hashi” and “relicode.com,” are owned by Patryk Szurgot iPapi. Nothing in these Terms transfers any intellectual property rights to you.
9. Disclaimer of Warranties
My Hashi is provided “as is” and “as available” without warranties of any kind, either express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, accuracy, or non-infringement.
Patryk Szurgot iPapi does not warrant that:
- The application will be uninterrupted, error-free, or free of viruses or other harmful components
- Any errors or defects in the application will be corrected
- The application will be compatible with all devices or operating system versions
10. Limitation of Liability
To the maximum extent permitted by applicable law, Patryk Szurgot iPapi shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of data, loss of profits, or personal injury, arising out of or in connection with your use of or inability to use My Hashi.
In jurisdictions where limitation of liability for personal injury or certain other damages is not permitted, Patryk Szurgot iPapi’s liability shall be limited to the maximum extent permitted by law.
11. Changes to the Application and Terms
Patryk Szurgot iPapi reserves the right to:
- Update, modify, or discontinue My Hashi at any time without prior notice
- Modify these Terms at any time
Significant changes to these Terms will be communicated through an in-app notice or an update to the “Last updated” date at the top of this document. Your continued use of the application after changes are made constitutes your acceptance of the revised Terms.
12. Termination
You may stop using My Hashi at any time by uninstalling it from your device. Patryk Szurgot iPapi may suspend or terminate your access to the application if you breach these Terms.
Upon uninstallation, all data stored locally on your device will be permanently deleted. This action is irreversible.
13. Governing Law and Dispute Resolution
These Terms are governed by and construed in accordance with the laws of Poland, without regard to its conflict of law provisions. Any disputes arising out of or relating to these Terms or the application shall be subject to the exclusive jurisdiction of the courts of Warsaw, Poland.
If you are a consumer residing in the European Union, you may also have the right to use the EU Online Dispute Resolution platform available at https://ec.europa.eu/consumers/odr.
14. Consumer Rights (EU / Poland)
If you are a consumer within the European Union or Poland, you retain all rights granted to you under applicable mandatory consumer protection laws, which these Terms do not override. In particular, consumers in Poland are protected under the Ustawa o prawach konsumenta (Consumer Rights Act) and relevant EU directives.
